Is my clownfish sick?

Hello I have had my reef tank for 5 months already and I got this clownfish about 1 month ago and I noticed that he started to rub himself on the sand bed. He has one white spot that looks like a pimple near his face and on the side he has like a couple little bumps.
Parameters:
High rang PH: 8.0
Ammonia: 0ppm
Nitrite: 0ppm
Nitrate: 5ppm

I would feed him garlic soaked foods and watch him closely. How are your parameters and do they fluctuate ?

My parameters are stable. The clown fish eats normally and swims normally as well only that I did noticed that sometimes he gets ichy and rubs himself on the sand
